Habitat for Humanity in Wayne County Golf Outing
waiver, release of liability and assumption of risk agreement
In order to participate in any capacity in the Habitat for Humanity in Wayne County Golf Outing on May 20, 2024 (October 7, 2024 rain date), hereinafter referred to as “HFH Golf Outing”, held by Habitat for Humanity in Wayne County at Wooster Country Club, 125 Oak Hill Rd, Wooster, OH 44691, the undersigned acknowledges, understands and agrees that:
- While golfing is an outstanding recreational sport, it also involves risk. Some of these risks include but are not limited to: traveling on or crossing heavily traveled pathways, winding pathways, steep descents, potholes, accidents, unexpected moves of another player/cart driver, physical exertion, and fatigue. The course terrain, pace, and distance may vary from the written or verbal description.
- The risk of injuries from activities involved in the HFH Golf Outing is significant, including the potential for permanent paralysis and death. While particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury and death does exist.
- I knowingly and freely assume all such risks, both known and unknown, and assume all and full responsibility for my participation. I willingly agree to comply with the stated and customary terms and conditions for participation and all rules, policies, and regulations of Wooster Country Club.
- I, for myself, and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Habitat for Humanity in Wayne County, their officers, agents, owners, employees, volunteers, lessors, other participants, sponsoring agencies, sponsors, and advertisers of the HFH Golf Outing with respect to any and all injury, disability, death, loss or damage to person or property, whether arising from the negligence of Habitat for Humanity in Wayne County or other entities and their representatives stated herein.
- I warrant that I am competent to play golf safely and that my equipment is in safe working condition. I agree to obey all course guidelines and to practice safety and courtesy when golfing.
- I understand that all persons will be responsible for carrying their own insurance, including a sports injury policy (medical).
I have read this waiver/release of liability/assumption of risk agreement, fully understand it’s terms, have consulted an attorney, or waived such rights, and I understand that I have given up substantial rights by signing it and signing it freely and voluntarily without any inducement.
